Supported Living

We know that nding the right care and support for your needs can be tricky, especially when you want to maintain your independence. That’s why our supported living options are designed around what’s important to you. Supported living offers an alternative to residential care and empowers you to choose the right amount of support – how and when you need it – so you can lead an enriching and fullling life. Our supported living is based on positive outcomes and expertise of developing environments that focus on promoting and encouraging the people we support’s independence and functioning. The type of support you receive is controlled by you and with our staff, you will develop a personalized support plan based on your interests, priorities and skills. From help with personal care needs and managing nances, to nding education, work or leisure activities – you decide! This can be in a variety of settings dependent on your needs, circumstances and goals but they could include any of the following:
High Needs
This is shared accommodation of usually 4-6 beds where you receive 24 hours care and support. Usually there is a sleep in or waking night support worker based on your needs. This may be ideal if you have been discharged from hospital and need to recover or you may need additional support due to anxiety, emotional distress or challenging behaviour and the need to keep safe.
Medium Needs
You could live in a smaller setting of shared accommodation of 2- 4 beds where you receive support when you need it. This may be from 4-12 hours per day and there is no overnight staff on site. This would be a structured service tom prepare you for independent living. You will have access to an on-call manager to support you when staff are not on site.
Low Needs (Independent Living)
You could choose to live independently in your own at with as much or as little support as you need. This could be in one of our already established properties, or we can work with you and our housing partners to source a property that meets your needs.
Shared Living
We provide shared accommodation for those individuals who want to live independently too but aren’t yet ready to live alone. You will have your own space but benet from socialising with other like-minded people, sharing the communal spaces and sharing the costs of household bills. This is run on a step down/recovery service model.
Apartment Living
You may be looking for the best of both worlds. Your own modern apartment with some shared communal space so you can choose when and if you want to socialise. We have worked with our housing partners to develop this popular type of accommodation.

DOMICILIARY CARE

From help with personal care needs and managing nances, to nding education, work or leisure activities – you decide! This can be in a variety of settings dependent on your needs, circumstances and goals but they could include any of the following:

  • Remind or assist you to take your medicines and collect or return medication from your pharmacy or dispensing GP surgery.
  • Prepare meals with or for you and assist you at mealtimes.
  • Collect your pension for you or with you.
  • Shop with you or help you to make a shopping list, go to the shops, come back and put it all away.
  • Help with your laundry or ironing and keeping your home clean and tidy.
  • Support you with social activities such as going out for a walk, attending a day centre, visiting friends or family, going to your church or club etc.
  • We also give a little pampering when you need it.
  • At the end of each day, some help with getting ready for bed.

 

Living-in Care
24-hour live-in care service could be the most reliable and cost-effective solution for you. Our live- in care service means your beloved one can get the care and support they need right in the comfort and security of their own home. We can provide a carer matched to the needs of your beloved one who would stay with them 24 hours a day, 7 days a week. We carefully handpick live-in carers who are suited to meet the needs of your beloved one and at the same time helping to promote and maintain their independence. We cater to a wide range of personal requirements, conditions and circumstances. Whether you want companionship and some help around your home or perhaps more specialised care for serious conditions like People with Disabilities, Dementia, Alzheimer’s or Parkinson’s or recuperating from a major surgery. Residential Care
Adults may be placed in Adult Residential Facilities because of a disability, often a mental disability which makes them unable to care for their daily needs. Various forms of long-term residential care are available for elderly people and young people over 18 years of age. They may choose to x their own meals or have meals provided, or some combination of both. Many residential facilities are designed to care for residents who do not need 24- hour nursing care but are unable to live independently. These are typically in providing a furnished or unfurnished room depending residents’ choice in a shared house, together with all meals and housekeeping and laundry service. Depending on the needs of the resident, we also provide assistance with daily activities such as personal hygiene, dressing, eating, and walking. These are not considered to be medical facilities, but they do have to meet state standards for care and safety.
Community Care
This service type provides a range of health care staff (e.g., nurses or allied health professionals) other than doctors to people who need health care support in their own home, in community settings or in development / rehabilitation units. The care provided may be short- or long-term and meet acute or chronic health care needs. The services may help people to live independently in the community and are directly responsible for the quality of the care and support provided by the staff they supply, and do not include employment agencies.
